Salmeterol falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.
Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.

Here's the concern with combining these two. Your inhaler, salmeterol (Serevent), can nudge the heart's electrical rhythm by lengthening what's called the QT interval. Iboga, through its main compound ibogaine, has been linked in several real-world case reports to that same kind of QT lengthening. Put them together and the effects may stack up, which can raise the risk of a dangerous irregular heartbeat.

I'll be honest, this evidence is largely theoretical and based on scattered case reports, not solid studies, so I'm not trying to scare you. But ibogaine is genuinely risky for the heart on its own (and it's illegal here in the U.S.). Please don't change how you use your salmeterol, and talk with your doctor or pharmacist before mixing it with anything like this.

Iboga is a Central African shrub whose root bark contains ibogaine, a powerful hallucinogenic compound studied mainly for addiction. It carries serious safety risks, including dangerous heart rhythm changes and deaths, and ibogaine is illegal in the United States and many other countries. We do not recommend its use outside of tightly controlled research settings.

What Is Salmeterol Oral Inhalation Used For?

Brand name: Serevent

Serevent Diskus (salmeterol inhalation powder) is used for three main purposes: treating asthma in patients aged 4 and older alongside an inhaled corticosteroid (ICS), preventing exercise-induced bronchospasm (EIB) in patients aged 4 and older, and providing long-term maintenance treatment of bronchospasm associated with COPD — including chronic bronchitis and emphysema — in adults.

Important: Serevent Diskus is not a rescue inhaler. It will not relieve a sudden asthma or COPD attack. For asthma, it must always be used together with an inhaled corticosteroid — using it alone for asthma is contraindicated due to a serious risk of asthma-related death.
