Why this applies to Dacomitinib

Dacomitinib falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS — the group Iboga interacts with.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to understand: in this pairing, Dacomitinib is the one doing the blocking, not the supplement. Dacomitinib slows down a liver enzyme called CYP2D6, and ibogaine (the active part of Iboga) happens to be cleared by that same enzyme. So if someone took them together, the Dacomitinib could cause ibogaine to build up to higher levels than expected.

That matters because Iboga is already risky on its own, especially for dangerous heart rhythm changes. Pushing its levels higher could make those effects worse. This is largely theoretical and based on limited study, but the direction is concerning enough that I wouldn't combine them. Iboga isn't something I'd recommend regardless, so please talk with your oncologist or pharmacist before mixing anything new with your Vizimpro.

Vizimpro (dacomitinib) is used as a first-line treatment for adults with metastatic non-small cell lung cancer (NSCLC) — meaning lung cancer that has spread — when the tumor carries a specific genetic change in the EGFR gene (an exon 19 deletion or exon 21 L858R substitution mutation). Before starting, your tumor must be tested with an FDA-approved test to confirm one of these mutations is present.
