Iboga interacts with whole categories of medication. Lofepramine falls into 3 of those categories, so it can interact with Iboga in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
QT INTERVAL-PROLONGING DRUGS

Lofepramine falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.
Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with these two together. Lofepramine is a tricyclic antidepressant, and like many in its class it can stretch out the heart's electrical cycle (what we call the QT interval). Iboga, through its main compound ibogaine, has been linked in real case reports to the same kind of QT stretching, sometimes with dangerous results. Put the two side by side and that effect can stack, raising the risk of a serious, irregular heartbeat.
To be fair, this is rated as theoretical and based on scattered case reports, not strong studies. But ibogaine already carries real heart-rhythm dangers on its own and isn't legal here. I'd never want you to mix them without talking to your doctor or pharmacist first.

ANTICHOLINERGIC DRUGS

In vitro research shows that iboga can inhibit acetylcholinesterase and might increase acetylcholine levels. However, the constituent ibogaine does not appear to have this effect.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this one. Lofepramine is a tricyclic antidepressant, and part of how it works (and part of why it causes side effects like dry mouth, blurred vision, constipation, and trouble urinating) is by blocking the chemical messenger acetylcholine. In lab dishes, Iboga seems to do the opposite, raising acetylcholine levels. So in theory, Iboga could nudge against some of lofepramine's effects.
I want to be honest about how thin this is. It comes from test-tube research only, and ibogaine, Iboga's main active compound, didn't even show this effect. There's no real-world proof it matters in people. Far more pressing is that Iboga itself carries serious heart-rhythm dangers and is illegal in the U.S. Please don't combine these without talking to your doctor or pharmacist first.

CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS
3 of 3Lofepramine also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the important thing to know with this pair: it's actually the Iboga that gets affected, not the other way around. Lofepramine is one of the antidepressants that can slow down the liver enzyme called CYP2D6. Ibogaine, the active compound in Iboga, relies on that same enzyme to be cleared from the body. So when Lofepramine is on board, ibogaine and its breakdown product can pile up. In one study, a similar CYP2D6 blocker doubled the exposure to ibogaine.
Why does that matter? Higher ibogaine levels raise the risk of its most dangerous effect, which is serious heart rhythm changes that have caused deaths. I want to be straight with you: ibogaine is illegal in the U.S. and isn't something I'd recommend outside tightly controlled research. Please don't combine these, and talk with your doctor or pharmacist about any concerns.
