Iboga and Trimipramine

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Iboga interacts with whole categories of medication. Trimipramine falls into 2 of those categories, so it can interact with Iboga in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

SEROTONERGIC DRUGS

1 of 2

Why this applies to Trimipramine

Trimipramine falls under SEROTONERGIC DRUGS — the group Iboga interacts with.

Theoretically, taking iboga with serotonergic drugs might increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Ibogaine, a constituent in iboga, has serotonergic effects. Theoretically, combining serotonergic drugs with iboga might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using iboga with serotonergic drugs.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: trimipramine (Surmontil) is a tricyclic antidepressant that nudges up serotonin in your brain. Iboga, through its compound ibogaine, can do the same thing. When you stack two things that both raise serotonin, you can get too much of it, a problem called serotonin syndrome. Warning signs include agitation, a racing heart, sweating, shivering, muscle twitching, and confusion.

I want to be honest with you: this concern is theoretical, based on how each one works rather than real case reports. But Iboga carries its own serious heart rhythm dangers, and trimipramine can also affect the heart, so the combination is genuinely worth avoiding. Please don't change anything on your own. Talk with your doctor or pharmacist first.

Moderate

QT INTERVAL-PROLONGING DRUGS

2 of 2

Trimipramine also falls under QT INTERVAL-PROLONGING DRUGS.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.
Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern with pairing Iboga and trimipramine. Trimipramine is a tricyclic antidepressant, and one of its known risks is that it can stretch out the heart's electrical cycle, something doctors call QT prolongation. Iboga (through its active compound ibogaine) has been linked in several case reports to the same kind of QT effect. Put them together and those effects may stack, which in rare cases can trigger dangerous, irregular heartbeats.

To be fair, this is mostly theoretical and based on scattered case reports, not strong studies. But ibogaine carries real heart risks on its own and is illegal in the U.S. Please don't combine these without talking to your doctor or pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Trimipramine page

“If you take trimipramine (Surmontil), a tricyclic antidepressant, read this list carefully. Nine of the 139 supplements and herbs on file are rated major, and most touch the heart: several, grapefruit among them, can stretch the QT interval, the heart's electrical reset time, and raise the risk of a dangerous rhythm.”

  • Opioid withdrawal
  • Spiritual and ceremonial rituals
  • Fatigue and stimulant effects
  • Mood and emotional issues

Iboga is a Central African shrub whose root bark contains ibogaine, a powerful hallucinogenic compound studied mainly for addiction. It carries serious safety risks, including dangerous heart rhythm changes and deaths, and ibogaine is illegal in the United States and many other countries. We do not recommend its use outside of tightly controlled research settings.

About Trimipramine

What Is Trimipramine Used For?

Brand names: Surmontil, Trimip, Rhotrimine, Tripramine, Trimp

Trimipramine capsules are indicated for the relief of symptoms of depression. Endogenous depression — depression that arises from within, rather than from an obvious outside cause — is more likely to respond to treatment than other types.

In clinical studies, trimipramine worked as well as imipramine in hospitalized patients with depression, and was comparable to amitriptyline in less severely depressed outpatients.
