Dronedarone falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.
Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.

Here's the concern with pairing these two. Dronedarone (Multaq) already stretches out the heart's electrical reset time, what we call the QT interval. Iboga, through its main compound ibogaine, has been linked in several real-world case reports to the same kind of QT lengthening. Put them together and those effects can stack up.

When the QT gets too long, it raises the risk of a dangerous, fast heart rhythm that can cause fainting or, rarely, sudden cardiac arrest. That's especially worrying because dronedarone is a heart-rhythm drug and iboga (ibogaine) is illegal in the U.S. with its own serious heart risks. This warning rests on anecdotal reports, not strong studies, but the danger is real enough that I'd urge you not to combine them. Please talk with your doctor or pharmacist first.

Dronedarone also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to understand: in this pairing, it's the Iboga that gets affected, not your Dronedarone. Ibogaine (the active part of Iboga) is broken down by a liver enzyme called CYP2D6, and Dronedarone happens to slow that enzyme down. So if someone took both, the ibogaine would clear more slowly and build up. One small study saw roughly a doubling of ibogaine exposure with another CYP2D6 blocker.

Why this matters so much here: both ibogaine and Dronedarone can affect the heart's rhythm and lengthen the QT interval. Stacking a higher ibogaine level on top of Dronedarone could meaningfully raise the risk of dangerous, even life-threatening, heart rhythm changes. Ibogaine is also illegal in the U.S. and unsafe outside research. Please don't combine these, and talk with your doctor or pharmacist about anything you're considering.

What Is Dronedarone Used For?

Brand name: Multaq

Multaq (dronedarone) tablets are used to reduce the risk of being hospitalized for atrial fibrillation (AF) — an irregular, often rapid heart rhythm. It is approved for adults who are currently in normal sinus rhythm but have a history of paroxysmal (comes and goes) or persistent atrial fibrillation.

Multaq is not appropriate for everyone with AF. It must not be used in people with permanent atrial fibrillation or in those with recently worsened heart failure requiring hospitalization — these patients face a doubled risk of death, stroke, and hospitalization if they take it.
