Escitalopram falls under SEROTONERGIC DRUGS — the group Iboga interacts with.

Theoretically, taking iboga with serotonergic drugs might increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Ibogaine, a constituent in iboga, has serotonergic effects. Theoretically, combining serotonergic drugs with iboga might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using iboga with serotonergic drugs.

Here's the gist: your Lexapro (escitalopram) already raises serotonin in the brain, which is how it eases depression and anxiety. Iboga, through its compound ibogaine, appears to push serotonin up too. Stack the two together and you could get too much serotonin activity, which is a problem called serotonin syndrome. Watch for things like agitation, a racing heart, sweating, shivering, muscle twitching, or confusion.

I'll be honest, this concern is mostly theoretical, based on how these substances work rather than firm case reports. But Iboga also carries real, serious heart rhythm risks of its own and is illegal in the U.S. Please don't combine them, and talk with your doctor or pharmacist before changing anything.

Escitalopram also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing with this pair: it's not your escitalopram (Lexapro) that builds up, it's the Iboga. Escitalopram mildly blocks an enzyme called CYP2D6, which is one of the main pathways your body uses to clear ibogaine, the active compound in Iboga. One small study found that a similar SSRI roughly doubled ibogaine exposure. So in plain terms, taking them together could leave more ibogaine in your system, raising the odds of its serious effects, especially dangerous heart rhythm changes.

On top of that, both raise serotonin, so combining them adds a risk of serotonin overload (agitation, sweating, racing heart, confusion). Iboga isn't something we recommend, and it's illegal in the U.S. Please don't mix these without talking to your doctor or pharmacist first.

What Is Escitalopram Used For?

Brand name: Lexapro

Escitalopram treats major depressive disorder (MDD) in adults and in adolescents 12 years of age and older. It also treats generalized anxiety disorder (GAD) in adults.

Lexapro (escitalopram) tablets are additionally approved to treat GAD in pediatric patients 7 years of age and older. Escitalopram capsules are indicated for MDD and GAD in adults younger than 65 years of age, and for MDD in pediatric patients 12 and older — but are not for use in adults 65 or older.
