Why this applies to Dronedarone

Dronedarone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rabdosia Rubescens interacts with.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the gist for Multaq (dronedarone). Your body relies on a liver enzyme called CYP3A4 to break this medicine down. Lab studies suggest a compound in Rabdosia rubescens (oridonin) may rev up, or induce, that enzyme. If that happened in real life, your body could clear dronedarone faster than usual, leaving less of it around. Since dronedarone isn't a prodrug, lower levels mean a weaker effect, which could let your atrial fibrillation slip back and raise your risk of a hospital visit.

Now the honest part: this is only test-tube and animal data, and it hasn't been seen in people. So I wouldn't panic. But because dronedarone is a heart-rhythm drug where steady levels matter, please run it by your pharmacist or doctor before combining the two.

What Is Dronedarone Used For?

Brand name: Multaq

Multaq (dronedarone) tablets are used to reduce the risk of being hospitalized for atrial fibrillation (AF) — an irregular, often rapid heart rhythm. It is approved for adults who are currently in normal sinus rhythm but have a history of paroxysmal (comes and goes) or persistent atrial fibrillation.

Multaq is not appropriate for everyone with AF. It must not be used in people with permanent atrial fibrillation or in those with recently worsened heart failure requiring hospitalization — these patients face a doubled risk of death, stroke, and hospitalization if they take it.
