Ranolazine falls under QT INTERVAL-PROLONGING DRUGS — the group Iboga interacts with.

Theoretically, iboga may have an additive effect with drugs that prolong the QT interval.
Numerous case reports suggest that ibogaine, a constituent in iboga, can cause QT interval prolongation.

Here's the concern with these two together. Ranolazine, your angina medication, is known to lengthen the QT interval, which is a measure of how long it takes your heart's electrical system to reset between beats. Iboga (through its active compound ibogaine) has been linked in several case reports to the same kind of QT lengthening. Stacking two things that push in the same direction can add up, and a longer QT can raise the risk of a dangerous, irregular heartbeat.

I want to be honest about the evidence: this is largely theoretical and based on scattered case reports, not strong studies. That said, ibogaine carries real heart-rhythm risks of its own and is illegal in the U.S. I'd steer clear of it, and please loop in your doctor or pharmacist before combining anything with ranolazine.

Ranolazine also falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here the concern runs the opposite way from what people usually expect. Ranolazine doesn't get supercharged by Iboga. Instead, ranolazine is a known blocker of the CYP2D6 enzyme, and that's the same enzyme your body uses to break down ibogaine (Iboga's active compound). With ranolazine slowing that cleanup, ibogaine and its byproduct noribogaine can pile up to higher levels, much like the study where a CYP2D6 blocker roughly doubled ibogaine exposure.

Why that matters: both ranolazine and Iboga can stretch the heart's QT interval and trigger dangerous rhythm problems. Stacking them is a real worry, not just theory. Iboga is also illegal in the U.S. and tied to deaths. Please don't combine these, and talk with your pharmacist or doctor first.

What Is Ranolazine Used For?

Brand names: Ranexa, Aspruzyo

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
