Why this applies to Darunavir

Darunavir falls under CYTOCHROME P450 2D6 (CYP2D6) INHIBITORS — the group Iboga interacts with.

Theoretically, taking iboga with CYP2D6 inhibitors might increase the levels and clinical effects of ibogaine.
Ibogaine, the active constituent of iboga, is a substrate of CYP2D6. One pharmacokinetic study shows that taking the CYP2D6 inhibitor paroxetine prior to taking ibogaine 20 mg increases the exposure to ibogaine and its metabolite noribogaine by 2-fold when compared with ibogaine alone.

Here's the key thing to understand: in this pairing, it's the iboga that gets affected, not your darunavir (Prezista). Darunavir, especially when boosted with ritonavir or cobicistat, blocks a liver enzyme called CYP2D6, which is the same enzyme that helps clear ibogaine, the active compound in iboga. With that pathway slowed down, ibogaine and its byproduct noribogaine can build up to roughly double the usual levels, based on a small study using a different CYP2D6 blocker.

Why this matters so much here: ibogaine is already known to cause dangerous heart rhythm problems and has been linked to deaths, so making those levels higher is genuinely risky. On top of that, ibogaine is illegal in the U.S. and isn't something we recommend. Please talk with your doctor or pharmacist before combining anything new with your HIV regimen.

About Darunavir

What Is Darunavir Used For?

Brand name: Prezista

Darunavir treats HIV-1 infection in adults and in children who are at least 3 years old and weigh at least 10 kg (about 22 lbs). It is not a cure for HIV, but it is used as part of a combination antiretroviral regimen to control the virus.

Darunavir must always be taken together with ritonavir — that pairing is required for darunavir to work properly. Both Prezista (brand name) and generic darunavir tablets are approved for this use. Prezista is also available as an oral suspension, which is particularly useful for younger or smaller patients.
