Chlorpheniramine, Hydrocodone, Pseudoephedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

This combination product (Tussend) has three pieces, and the part to focus on here is the hydrocodone, the cough and pain ingredient. Hydrocodone is actually a prodrug for some of its effect: the liver enzyme CYP2D6 converts a portion of it into hydromorphone, a stronger painkiller. Lab research hints that lentinan may speed up CYP2D6. If that happened in real people, it could mean a bit more of that active form, so in theory you might notice more drowsiness, dizziness, or constipation.

Honestly though, this is only test-tube and animal data, never confirmed in humans, so I wouldn't lose sleep over it. Still, since hydrocodone is an opioid, it's worth a quick chat with your pharmacist or doctor before pairing the two.

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for this combination product (Tussend), which pairs the cough medicine hydrocodone with the antihistamine chlorpheniramine and the decongestant pseudoephedrine. Lab and animal studies hint that lentinan may speed up a liver enzyme called CYP3A4. Hydrocodone leans on that enzyme to turn into its active pain and cough-suppressing form. So in theory, a faster enzyme could shift how much active drug you get, and it might also nudge how well the cough and pain relief work.

I want to be honest though: this has only been seen in test tubes and animals, never in real patients, so it's far from proven. Everyday shiitake mushrooms in food are not a concern. Before adding a concentrated lentinan supplement, just check with your pharmacist or doctor first.
