Why this applies to Remdesivir

Remdesivir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist with shiitake-derived lentinan and remdesivir (Veklury). Lab studies hint that lentinan may speed up an enzyme called CYP3A4, which is one of the pathways that helps clear remdesivir. If that enzyme runs faster, your body could break the antiviral down a bit quicker, which in theory might leave you with slightly less of it working against the virus.

I want to keep this in perspective, though. This is only test-tube and animal data, and it has never actually been seen in real patients. On top of that, remdesivir is given as an IV drip in a hospital or clinic for a short course, so a once-or-twice supplement is unlikely to derail treatment. Still, since you'd be sick enough to need it, let your care team know about any supplements so they can keep an eye on things.

What Is Remdesivir Injection Used For?

Brand name: Veklury

Veklury (remdesivir) is given by IV infusion to treat COVID-19 caused by the SARS-CoV-2 virus. It is approved for adults and for children from birth through age 17 who weigh at least 1.5 kg (about 3.3 lbs).

Veklury can be used for patients who are hospitalized with COVID-19, or for patients who are not hospitalized but have mild-to-moderate COVID-19 and are at high risk of getting worse — including being hospitalized or dying from the disease.
