Why this applies to Saquinavir

Saquinavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for saquinavir (Invirase). Your body clears this HIV medicine mainly through a liver enzyme called CYP3A4. Lab research hints that lentinan might speed that enzyme up. If that happened in people, your body could break saquinavir down faster, which may lower the drug levels in your blood. With an HIV protease inhibitor, weaker levels are a real concern because the goal is to keep the virus fully suppressed, and dropping levels could let HIV bounce back or build resistance.

That said, this is only seen in test-tube and animal studies, never reported in actual patients, so the evidence is weak. Please don't stop or change anything on your own. Just loop in your pharmacist or HIV doctor before adding lentinan so they can keep an eye on things.

What Is Saquinavir Used For?

  • Saquinavir is used in combination with ritonavir (Norvir) and other medications to treat human immunodeficiency virus (HIV) infection. Saquinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although saquinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
