Why this applies to Ruxolitinib
Ruxolitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with these two together. Your body relies on a liver enzyme called CYP3A4 to clear ruxolitinib (Jakafi or Opzelura). In test-tube studies, lentinan seems to speed that enzyme up. If that held true in real life, your body could break down ruxolitinib a little faster, which might leave less of it working. For something treating myelofibrosis or polycythemia vera, weaker drug levels could mean less control of your symptoms, like a creeping return of fatigue, an enlarging spleen, or shifting blood counts.
The good news: this is only seen in lab research so far, never reported in actual patients, so it stays theoretical. Still, since ruxolitinib needs steady dosing, please check with your oncologist or pharmacist before adding lentinan.

Ruxolitinib is used in two very different ways. Oral products (Jakafi and Jakafi XR) treat certain blood and bone marrow conditions and graft-versus-host disease. A topical cream (Opzelura) treats certain skin conditions.
Each use belongs to a specific product. Jakafi/Jakafi XR are not skin treatments, and Opzelura is not approved for the blood conditions.
