Ruxolitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.

Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.

Here's the thing to know about ruxolitinib (Jakafi, Opzelura): your liver clears it using an enzyme called CYP3A4. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme. In theory, that means your body could clear ruxolitinib a little faster, leaving you with somewhat lower levels of the medicine.

For a cancer treatment like this, weaker drug levels could mean it doesn't control your condition as well. I'll be honest though, this concern is theoretical and based on how the drugs work, not on real patient reports, and modafinil is only a weak inducer. Still, ruxolitinib is too important to leave to guesswork. Please check with your oncologist or pharmacist before pairing these.

About Ruxolitinib

What Is Ruxolitinib Used For?

Brand names: Jakafi, Opzelura

Ruxolitinib is approved to treat several serious blood and immune conditions in adults, and some in patients as young as 12. It is used for myelofibrosis (a bone marrow disorder that disrupts normal blood cell production), polycythemia vera (a condition where the bone marrow makes too many red blood cells), and two forms of graft-versus-host disease (GVHD) — a complication that can occur after a stem cell transplant.

For polycythemia vera, it is specifically for patients who have not responded well to, or cannot tolerate, a medication called hydroxyurea. For chronic GVHD, it is used after at least one or two other treatments have failed.
