Why this applies to Tamoxifen Citrate

Tamoxifen Citrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's something worth knowing about tamoxifen: it's actually a prodrug. Your body has to convert it into its active form (called endoxifen) using a liver enzyme named CYP2D6. That active form is what does the real work against breast cancer.

Lab research hints that lentinan might speed up CYP2D6. In theory, faster activity could change how much active endoxifen you end up with, and anything that lowers your effective tamoxifen could mean less protection against cancer recurrence. That's the concern here. But to be honest, this comes only from test-tube and animal studies, and it hasn't been seen in real patients. Because tamoxifen is so important, I'd still loop in your oncologist or pharmacist before adding lentinan, just to be safe.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the thing worth knowing about pairing lentinan with tamoxifen. Lab studies hint that lentinan may speed up an enzyme called CYP3A4, which is one of the liver enzymes that helps process tamoxifen. If that enzyme works faster, your body could clear tamoxifen a bit quicker, which in theory might lower the amount working to protect against your breast cancer.

I want to be honest about how shaky this is: it has only been seen in test-tube and animal research, never in real patients, so I'm not losing sleep over it. Still, because tamoxifen is doing such an important job, I wouldn't add a concentrated lentinan supplement without first checking with your oncologist or pharmacist. Eating shiitake mushrooms as food is a different story and very unlikely to matter.

Lentinan is a beta-glucan polysaccharide from shiitake mushrooms that is studied mainly as an immune-modulating add-on to cancer chemotherapy, where it is given by injection in a few countries. Evidence for oral supplements is limited, and quality varies widely between products. About Tamoxifen

What Is Tamoxifen Used For?

Brand names: Nolvadex, Soltamox

Tamoxifen is used to treat estrogen receptor-positive metastatic breast cancer in both women and men, and as follow-up (adjuvant) therapy after surgery for early-stage breast cancer in women. It also reduces the occurrence of cancer in the opposite breast when used as adjuvant therapy.

In women with ductal carcinoma in situ (DCIS — an early, non-invasive breast change) following surgery and radiation, tamoxifen lowers the risk of developing invasive breast cancer. It is also approved to reduce the risk of breast cancer in women who are at high risk but have not yet been diagnosed. Both Tamoxifen Citrate tablets and Soltamox oral solution share these indications.
