Tolterodine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist for Tolterodine (Detrol). Your liver uses an enzyme called CYP2D6 to break tolterodine down. Lab research hints that lentinan might speed that enzyme up. If that happened in real life, your body could clear tolterodine a little faster, which might make it work less well, so you could notice more of the overactive bladder symptoms you're treating, like urgency or frequent bathroom trips.

I want to keep this in perspective though. This is only a test-tube and animal finding, and it has never actually been reported in people. So it's more of a theoretical heads-up than a real-world warning. Still, since product quality varies a lot, it's worth mentioning lentinan to your pharmacist or doctor before adding it.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Tolterodine — available as tablets (including Detrol and Tolterodine Tartrate tablets) and extended-release capsules (including Detrol LA and Tolterodine Tartrate extended-release capsules) — is prescribed for overactive bladder. It targets three main symptoms: a sudden, hard-to-control urge to urinate (urgency), urinating too often (frequency), and leaking urine when that urge hits (urge urinary incontinence).
