Why this applies to Agomelatine

Agomelatine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.

Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the gist for agomelatine (Valdoxan). Your body relies heavily on an enzyme called CYP1A2 to break this antidepressant down. Lentinan, in early lab work, looks like it may rev that enzyme up a bit. If that happened in real life, your body could clear agomelatine faster, leaving less of it around to do its job. That might mean your depression symptoms aren't as well controlled.

The honest truth, though, is that this is only seen in test tube and animal studies, never reported in actual people, so I wouldn't lose sleep over it. Still, agomelatine already needs careful handling, so loop in your pharmacist or doctor before adding lentinan rather than changing anything on your own.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2C19 (CYP2C19). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2C19 in patients taking lentinan.
Some drugs metabolized by CYP2C19 include proton pump inhibitors including omeprazole (Prilosec), lansoprazole (Prevacid), and pantoprazole (Protonix); diazepam (Valium); carisoprodol (Soma); nelfinavir (Viracept); and others.

Here's the gist for agomelatine (Valdoxan). This antidepressant is mostly cleared by a liver enzyme called CYP1A2, with CYP2C19 playing a smaller, secondary role. Lab and animal research hints that lentinan may speed up CYP2C19. If that held true in people, your body could clear agomelatine a touch faster, which in theory might make it work a little less well for your mood or sleep.

That said, this is very early evidence, only seen in test tubes and animals, not in actual patients, and CYP2C19 isn't even agomelatine's main pathway. So I wouldn't lose sleep over it. Don't start or stop anything on your own. Just give your pharmacist or prescriber a heads-up so they can keep an eye on how well your treatment is working.
