Tramadol, Acetaminophen falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.

Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP1A2 in patients taking lentinan.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.

Here's the honest picture with Tramacet (tramadol plus acetaminophen) and lentinan. The concern on file is that lentinan might rev up a liver enzyme called CYP1A2. But this comes only from lab and animal studies, and it has never actually been seen in people, so I'd file it as a theoretical worry, not a red alarm.

For Tramacet specifically, CYP1A2 isn't the main player. Tramadol mostly relies on CYP2D6 and CYP3A4, and acetaminophen is handled through other routes. So even if lentinan nudges CYP1A2, it's unlikely to meaningfully change your pain relief or side effects like drowsiness or nausea. Still, since lentinan products vary a lot, it's worth a quick chat with your pharmacist or doctor before combining them.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the wrinkle with this combo: the tramadol part of Tramacet is actually a prodrug. Your body uses an enzyme called CYP2D6 to convert tramadol into the form that does most of the pain relief. Lab research hints that lentinan might speed up that same enzyme. If that happened in people, it could shift how tramadol is handled, which is why it shows up as a flag worth knowing about.

Honestly though, this is only a theoretical concern right now. It comes from test-tube and animal work, not from real patients, so I wouldn't lose sleep over it. The acetaminophen side isn't really involved here. If you're considering lentinan, just loop in your pharmacist or doctor first so they can keep an eye on your pain control.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 3A4 (CYP3A4). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking lentinan.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the wrinkle with Tramacet: the tramadol part is actually a prodrug. Your body has to convert it into its active painkilling form, and CYP3A4 is one of the enzymes involved in handling it. Lab studies hint that lentinan might speed up CYP3A4. If that happened in people, tramadol could be cleared a bit faster, which might mean less pain relief than you expect.

I want to be honest about how thin this evidence is. It comes only from test-tube and animal research, with nothing reported in real patients, so I wouldn't lose sleep over it. The acetaminophen side isn't really affected here. Still, if your pain control seems off after starting lentinan, mention it to your pharmacist or doctor before changing anything.
