Why this applies to Ponatinib
Ponatinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea with ponatinib (Iclusig). Your body relies on a liver enzyme called CYP3A4 to clear this drug. Lab studies hint that lentinan from shiitake mushrooms might speed that enzyme up. If that happened in real life, your body could break down ponatinib a little faster, leaving less of it to do its job against the leukemia cells. Ponatinib isn't a prodrug, so faster clearance simply means weaker treatment, not stronger.
I want to be honest about the evidence though: this is only seen in test-tube and animal work, never confirmed in people, so it's more of a theoretical caution than a proven problem. Still, because ponatinib is such an important cancer medicine, please run any lentinan supplement past your oncologist or pharmacist first.

What Is Ponatinib Used For?
Brand name: Iclusig
Iclusig (ponatinib) is used to treat adult patients with Philadelphia chromosome-positive acute lymphoblastic leukemia (Ph+ ALL) — either newly diagnosed (in combination with chemotherapy) or in patients for whom no other kinase inhibitors are appropriate, or who have a specific mutation called T315I. It is also used for adults with chronic myeloid leukemia (CML), including chronic phase, accelerated phase, and blast phase CML, in situations where prior treatments have not worked or are not tolerated, or where the T315I mutation is present.
Iclusig is not recommended for patients newly diagnosed with chronic phase CML who have not yet tried other treatments.
