Why this applies to Perphenazine

Perphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Lentinan interacts with.

Laboratory research suggests that lentinan might increase the activity of cytochrome P450 2D6 (CYP2D6). So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP2D6 in patients taking lentinan.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the gist for perphenazine (Trilafon). Your liver uses an enzyme called CYP2D6 to break this medicine down. Lab studies hint that lentinan might speed that enzyme up. If that happened in real life, your body could clear perphenazine a little faster, leaving less in your system. The practical worry would be your symptoms (the disturbed thinking or the severe nausea you're treating) creeping back because the drug isn't working as well.

Now the honest part: this is only seen in test-tube and animal research, never confirmed in people, so I wouldn't lose sleep over it. Don't change your dose on your own. If you'd like to try lentinan, just loop in your pharmacist or doctor first so they can keep an eye on how well your perphenazine is working.

Perphenazine tablets are approved to treat schizophrenia in adults. They are also used to control severe nausea and vomiting in adults when other measures are not enough.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
