Perphenazine falls under ANTIPSYCHOTIC DRUGS — the group Manganese interacts with.

Theoretically, the risk for manganese toxicity might increase when taken with antipsychotic drugs.
Hallucinations and behavioral changes have been reported in a patient with liver disease who was taking haloperidol and manganese. Researchers speculate that taking manganese along with haloperidol, phenothiazine-derivatives, or other antipsychotic medications might increase the risk of manganese toxicity in some patients.

Here's the gist with perphenazine (Trilafon) and a manganese supplement. This isn't a case of one changing the blood level of the other. Instead, the concern is that both perphenazine and extra manganese can affect the same movement-control areas of the brain. The worry is that piling them together might make manganese harder for your body to handle, raising the small risk of manganese building up.

Honestly, this is mostly theoretical and comes from a single report in someone with liver disease, so it's weak evidence. Food and a regular multivitamin amount of manganese are very unlikely to matter. Concentrated, high-dose manganese is where I'd be cautious, since both can contribute to stiffness, tremor, or restlessness. Please check with your pharmacist or doctor before adding it.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
