Here's the honest picture for this cold and cough combo. The concern on file is that lentinan might speed up a liver enzyme called CYP1A2, which could lower blood levels of drugs cleared that way. Of the four ingredients in this product, none of them lean heavily on CYP1A2. Acetaminophen, chlorpheniramine, dextromethorphan, and pseudoephedrine are mostly handled by other pathways, so this particular interaction is unlikely to cause you trouble.
On top of that, the evidence is only from lab and animal work, not from real people, so I wouldn't lose sleep over it. None of these are prodrugs that would flip the effect either. Still, since lentinan products vary a lot, it's worth a quick word with your pharmacist before combining them, especially if you take it regularly.

Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

This cold and cough combo has one ingredient that matters most here: dextromethorphan, the cough suppressant. Your body relies on an enzyme called CYP2D6 to clear it. Lab research hints that lentinan might speed that enzyme up a bit, which in theory could clear dextromethorphan a little faster and make it slightly less effective for quieting a cough. The other ingredients (acetaminophen, chlorpheniramine, and pseudoephedrine) aren't really driven by this pathway, so they're not the concern.
Honestly, this is only seen in test-tube and animal studies, never in real people, so I wouldn't lose sleep over it. If your cough just isn't easing up while taking lentinan, that's worth mentioning to your pharmacist or doctor. Don't change anything on your own.

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

Here's the gist for this multi-symptom cold medicine. Lab studies hint that lentinan might speed up an enzyme called CYP3A4, which helps break down a couple of the ingredients here, mainly dextromethorphan (the cough suppressant) and to some extent chlorpheniramine (the antihistamine). If that enzyme runs faster, those ingredients could clear a bit quicker, so in theory you might notice slightly less cough relief or less drowsiness.
Honestly, I wouldn't lose sleep over this one. The evidence is only from test-tube and animal research, and it has never actually been reported in people. The acetaminophen and pseudoephedrine pieces don't really lean on this pathway anyway. Still, if you take lentinan regularly, it's worth a quick word with your pharmacist or doctor.
