Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Cariprazine Hydrochloride
Cariprazine Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lentinan interacts with.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version for Vraylar (cariprazine). Your liver uses an enzyme called CYP3A4 to process this medicine. Lab studies hint that lentinan might speed that enzyme up, which could clear cariprazine out a bit faster and lower its levels. In plain terms, your medication might not work as well, so you could notice mood symptoms, anxiety, or psychosis creeping back in.
I want to be honest with you though: this is based only on test tube and animal research, and it has never actually been seen in people. So it's more of a theoretical caution than a proven problem. Cariprazine also has long-acting active byproducts that linger for weeks, which softens any quick swing. Before adding lentinan, just run it by your pharmacist or doctor so they can keep an eye on how you're feeling.

From our pharmacist’s overview on the Cariprazine Hydrochloride page
“Cariprazine (Vraylar), an antipsychotic prescribed for schizophrenia and bipolar disorder, is a medication I would pair with supplements only carefully, mostly because the liver enzyme that clears it, CYP3A4, is easy to disturb. The great majority of the 145 flagged ingredients either block that enzyme and let cariprazine build up in t…”
