Perphenazine falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Thuja interacts with.

Thuja products can contains significant amounts of thujone, a neurotoxin. Theoretically, patients taking drugs that lower the seizure threshold might be at greater risk of seizure if they also take thuja. Advise patients taking these drugs to avoid thuja products. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the concern with combining thuja and perphenazine (Trilafon). Concentrated thuja products, especially the oil, contain thujone, which is a nerve toxin that can make the brain more prone to seizures. Perphenazine is an older antipsychotic that, on its own, can already lower the seizure threshold a bit. Stacking the two together could, in theory, raise the chance of a seizure.

I want to be honest about how strong this evidence is: it's mostly theoretical and anecdotal, not proven in solid studies. Still, because the safest move is easy here, I'd steer clear of concentrated thuja supplements and oil while you're on perphenazine. Please run it by your pharmacist or doctor before adding any new product.

Perphenazine tablets are approved to treat schizophrenia in adults. They are also used to control severe nausea and vomiting in adults when other measures are not enough.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
