Tramadol falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Thuja interacts with.

Thuja products can contains significant amounts of thujone, a neurotoxin. Theoretically, patients taking drugs that lower the seizure threshold might be at greater risk of seizure if they also take thuja. Advise patients taking these drugs to avoid thuja products. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's something worth knowing about tramadol and thuja. Tramadol already carries a known risk of lowering your seizure threshold, meaning it can make seizures a little more likely, especially at higher doses or when combined with other things that do the same. Thuja, particularly the oil, contains thujone, a compound that can act as a neurotoxin and may also nudge the seizure threshold down.

Put them together and the concern is that the two effects could add up, slightly raising your chance of a seizure. I want to be honest: this is based on weak, theoretical evidence rather than strong studies. Still, since tramadol is involved, I'd play it safe and avoid concentrated thuja products. Please check with your pharmacist or doctor first.
