Phenytoin falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Thuja interacts with.

Thuja products can contains significant amounts of thujone, a neurotoxin. Theoretically, patients taking drugs that lower the seizure threshold might be at greater risk of seizure if they also take thuja. Advise patients taking these drugs to avoid thuja products. Some drugs that lower the seizure threshold include anesthetics (propofol, others), antiarrhythmics (mexiletine), antibiotics (amphotericin, penicillin, cephalosporins, imipenem), antidepressants (bupropion, others), antihistamines (cyproheptadine, others), immunosuppressants (cyclosporine), narcotics (fentanyl, others), stimulants (methylphenidate), theophylline, and others.

Here's the thing with Thuja and your phenytoin (Dilantin). Concentrated Thuja products can contain a fair amount of thujone, which is a nerve toxin that may actually make seizures more likely. Since you take phenytoin specifically to keep seizures under control, that's working against the very thing your medicine is trying to do. This isn't about Thuja changing your phenytoin levels in the blood, it's about Thuja possibly lowering your seizure threshold.

I'll be honest that this concern is mostly theoretical and based on anecdotal reports, not strong human studies. Still, with a seizure medicine where steady control really matters, I'd play it safe. Please check with your pharmacist or doctor before using any Thuja product.

Here's the concern with pairing Thuja and your phenytoin (Dilantin). Thuja can contain a compound called thujone, and in higher amounts thujone may make the brain a little more prone to seizure activity. Since phenytoin's whole job is to calm down that abnormal electrical activity, anything pulling in the opposite direction could, in theory, make your medication less effective and allow a breakthrough seizure.

I want to be honest about how strong this is: it's based on anecdotal reports and the known properties of thujone, not solid human studies, so it's more of a theoretical worry than a proven one. Still, because seizure control is so important, I'd skip concentrated Thuja oil and check with your doctor or pharmacist before using any Thuja product.

Thuja, also called northern white cedar or arborvitae, is a tree whose leaf and oil have a long history in folk and homeopathic medicine, especially for warts and immune support. Solid human evidence for these uses is very limited, and the undiluted oil contains thujone, which can be toxic. Talk with a pharmacist or doctor before using it, and avoid it during pregnancy and breastfeeding.

About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
