Here is the interaction we found between these two, with the evidence behind it.
PHENOTHIAZINES
Why this applies to Perphenazine
Perphenazine falls under PHENOTHIAZINES — the group Coffee interacts with.
Coffee contains caffeine. Phenothiazines can reduce the metabolism of caffeine by inhibiting cytochrome P450 1A2 (CYP1A2). Additionally, the tannins in coffee may bind to phenothiazines in the gastrointestinal tract and reduce their absorption.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the practical takeaway for perphenazine (Trilafon) and your morning coffee. There are two small things going on. First, perphenazine can slow how your body breaks down caffeine, so the same cup might hit a little harder, leaving you more wired, jittery, or restless than usual. Second, the tannins in coffee can latch onto perphenazine in your stomach and slightly reduce how much you absorb, which in theory could make the medication work a bit less well.
Honestly, this is rated minor and is based only on lab-type evidence, so for most people moderate coffee isn't a big deal. A simple trick is to space your coffee an hour or two apart from your dose. If you notice more jitters or feel your symptoms aren't as controlled, check in with your pharmacist or doctor.
