Coffee contains caffeine. There is evidence that using ephedrine with caffeine might increase the risk of serious life-threatening or debilitating adverse effects such as hypertension, myocardial infarction, stroke, seizures, and death. Tell patients to avoid taking caffeine with ephedrine and other stimulants.

This old combination medicine (Amesec) contains ephedrine, a stimulant, plus aminophylline, which is closely related to caffeine. Coffee adds even more caffeine on top of all that. When you stack stimulants like this, the effects can pile up and put real strain on your heart and nervous system.

What that can look like in everyday terms: a racing or pounding heartbeat, jitteriness, trouble sleeping, anxiety, tremor, and a jump in blood pressure. The serious end (very high blood pressure, heart rhythm problems, or seizures) is rare but has been reported. The evidence here is mostly anecdotal, but the concern is real enough that I'd play it safe. Please check with your pharmacist or doctor before pairing coffee with this medicine.

Here's the thing to keep in mind with Amesec: it actually packs two stimulants of its own. The aminophylline (a theophylline product) and the ephedrine both rev up your nervous system, and caffeine in your coffee does the same job through a similar pathway. So when you stack your morning cups on top of this medicine, you can essentially be doubling down on stimulation.

What that looks like in real life is a racing or pounding heart, jitteriness, shaky hands, trouble sleeping, headache, or feeling wired and anxious. This is mostly an add-on effect rather than coffee changing how the drug is broken down, and it's based on expert opinion rather than hard studies. Still, if you notice your heart fluttering or you feel overly keyed up, ease off the coffee and check in with your pharmacist or doctor.
