Coffee contains caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, the caffeine in coffee might increase the risk of bleeding when used concomitantly with these agents. However, this interaction has not been reported in humans. There is some evidence that caffeinated coffee might increase the fibrinolytic activity in blood.

Here's the situation with your inhaled iloprost (Ventavis) and your morning coffee. Iloprost does more than relax the blood vessels in your lungs, it also has a mild effect of keeping platelets from clumping together, which is part of how it works. Caffeine in coffee has been reported to have a small platelet-slowing effect of its own, so in theory the two together could nudge up the chance of bleeding or easy bruising.

I want to be honest about how soft this evidence is. This comes from lab and test-tube studies only, and it has never actually been seen in real people. Normal coffee drinking is very unlikely to cause a problem. Still, if you notice unusual bruising, nosebleeds, or bleeding that won't stop, let your doctor or pharmacist know.
