Coffee and Trimipramine

Here is the interaction we found between these two, with the evidence behind it.

Minor

TRICYCLIC ANTIDEPRESSANTS (TCAs)

Why this applies to Trimipramine

Trimipramine falls under TRICYCLIC ANTIDEPRESSANTS (TCAs) — the group Coffee interacts with.

Theoretically, TCAs might bind with coffee constituents when taken at the same time.
In vitro research suggests that the tannins in coffee might cause precipitation of TCAs. However, this effect has not been reported in humans.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: this one is mostly a lab curiosity, not something that's actually shown up in real patients. In a test tube, the natural tannins in coffee can bind to tricyclic antidepressants like trimipramine and cause them to clump up (precipitate). In theory, that could mean a little less of your dose gets absorbed if the two mix right in your stomach at the same time.

The thing is, this has never actually been reported in people, so I wouldn't lose sleep over it. If you want to play it safe, just put a little space between your morning coffee and your trimipramine dose. And since trimipramine is one you don't want swinging around too much (too little may dull its effect on your mood; too much can bring drowsiness, dry mouth, or dizziness), check in with your pharmacist before changing anything.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Trimipramine page

“If you take trimipramine (Surmontil), a tricyclic antidepressant, read this list carefully. Nine of the 139 supplements and herbs on file are rated major, and most touch the heart: several, grapefruit among them, can stretch the QT interval, the heart's electrical reset time, and raise the risk of a dangerous rhythm.”

About Coffee

What is Coffee?

  • Alertness and reducing fatigue
  • Mental focus and concentration
  • Athletic performance
  • Headache relief
  • Weight management

Coffee is a widely consumed beverage made from roasted coffee beans, valued mainly for its caffeine, which boosts alertness and energy. For most healthy adults, moderate coffee intake is generally safe and may have some health benefits, but too much can cause jitteriness, poor sleep, and a faster heartbeat. People who are pregnant, breastfeeding, or sensitive to caffeine should limit their intake and check with a doctor or pharmacist.

About Trimipramine

What Is Trimipramine Used For?

Brand names: Surmontil, Trimip, Rhotrimine, Tripramine, Trimp

Trimipramine capsules are used to relieve the symptoms of depression. Both supported products (Trimipramine and Trimipramine Maleate oral capsules) carry this same use.

Depression with an “endogenous” pattern is more likely to improve than other depressive states. In the label’s studies, it worked about as well as amitriptyline in less-depressed outpatients, but somewhat less well in more severely depressed ones. In hospitalized patients, it worked about as well as imipramine.
