Coffee and Clomipramine

Here is the interaction we found between these two, with the evidence behind it.

Minor

TRICYCLIC ANTIDEPRESSANTS (TCAs)

Why this applies to Clomipramine

Clomipramine falls under TRICYCLIC ANTIDEPRESSANTS (TCAs) — the group Coffee interacts with.

Theoretically, TCAs might bind with coffee constituents when taken at the same time.
In vitro research suggests that the tannins in coffee might cause precipitation of TCAs. However, this effect has not been reported in humans.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with coffee and your clomipramine (Anafranil). In a test tube, the tannins found in coffee can latch onto tricyclic antidepressants like clomipramine and make them clump together, or precipitate. The worry is that this could keep some of the dose from being absorbed, which in theory might make the medicine work a bit less well for your OCD symptoms.

The important part: this is only lab-dish data, and it has never actually been seen in real people. So I wouldn't lose sleep over it. If you want to be cautious, just don't wash your pill down with a cup of coffee. Take it with water and space them apart a little. Any questions about timing, your pharmacist is happy to help.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Clomipramine page

“A tricyclic antidepressant used mostly for OCD, clomipramine (Anafranil) is unforgiving of shifts in its blood level, which is why its 236 supplement interactions deserve genuine wariness. Most of the list involves metabolism: herbs that speed up or slow down the liver enzymes handling clomipramine can drag its level too low or push it…”

  • Weight management

Coffee is a widely consumed beverage made from roasted coffee beans, valued mainly for its caffeine, which boosts alertness and energy.
