Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: repaglinide works by nudging your pancreas to release insulin, which brings your blood sugar down. The caffeine in coffee can have a small, unpredictable effect on blood sugar, sometimes raising it a little and sometimes lowering it. That's why this pairing carries only a minor rating, and the evidence behind it is fairly weak and mostly theoretical.

In real life, your morning cup of coffee is very unlikely to throw off your repaglinide. The main thing I'd watch for is whether large amounts of caffeine make it harder to read your numbers, since signs like a racing heart or feeling shaky can overlap with a low blood sugar reaction. If you drink a lot of coffee or use caffeine pills, mention it to your pharmacist or doctor so they can help you keep an eye on your readings.

Nearly everything on it also acts on blood sugar, usually pushing it lower still and raising the odds of a hypoglycemic crash, though a handful, niacin among them, pull the other way a…”
