These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Coffee interacts with whole categories of medication. Ephedrine, Hydroxyzine, Theophylline falls into 3 of those categories, so it can interact with Coffee in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Major).
EPHEDRINE
1 of 3Coffee contains caffeine. There is evidence that using ephedrine with caffeine might increase the risk of serious life-threatening or debilitating adverse effects such as hypertension, myocardial infarction, stroke, seizures, and death. Tell patients to avoid taking caffeine with ephedrine and other stimulants.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing about this combination pill: it contains two stimulants already. Ephedrine revs up your heart and blood pressure, and theophylline is chemically a cousin of caffeine that does much the same. When you add coffee on top, you're basically stacking caffeine onto two drugs that are already pushing your system in the same direction.
That can mean a racing or pounding heart, jitteriness, trouble sleeping, anxiousness, or a spike in blood pressure. In rare cases the combined load has been linked to more serious events. Theophylline is also a tricky drug with a narrow safe range, so changes matter. The strong proof here is mostly anecdotal, but it's worth respecting. Please check with your pharmacist or doctor about how much coffee is okay for you.

STIMULANT DRUGS
2 of 3Why this applies to Ephedrine, Hydroxyzine, Theophylline
Ephedrine, Hydroxyzine, Theophylline falls under STIMULANT DRUGS — the group Coffee interacts with.
Coffee contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
This combination pill already packs a punch. Both ephedrine and theophylline are stimulants in their own right, and theophylline is actually chemically related to caffeine. So when you add coffee on top, you're really stacking stimulant on stimulant. That can add up to a faster or pounding heartbeat, jitteriness, shakiness, anxiety, trouble sleeping, and an upset stomach.
One thing worth knowing: theophylline has a fairly narrow safety window, and large amounts of caffeine can nudge its levels and effects a bit higher. This is based mostly on expert opinion rather than hard studies, so a small daily cup likely won't cause trouble. Still, if you notice your heart racing or feel wired, mention it to your pharmacist or doctor before changing anything.

THEOPHYLLINE
3 of 3Ephedrine, Hydroxyzine, Theophylline also falls under THEOPHYLLINE.
Coffee contains caffeine, which can increase theophylline levels.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
This combo product (Marax) packs a few punches at once, but the piece that matters with coffee is the theophylline. Theophylline is chemically very close to caffeine, and your body actually clears them through some of the same liver pathways. When you add the caffeine from coffee on top, it can slow theophylline's breakdown and let its levels creep up.
Theophylline has a narrow safety window, so even a modest bump can tip you into side effects like a racing or pounding heart, jitteriness, nausea, trouble sleeping, or feeling shaky. The ephedrine in this product is a stimulant too, so the caffeine just stacks onto that. This is based on limited evidence, not strong proof, so don't panic. A cup or two may be fine for many people, but it's worth asking your pharmacist or doctor how much coffee is okay for you.

- Athletic performance
- Headache relief
- Weight management
Coffee is a widely consumed beverage made from roasted coffee beans, valued mainly for its caffeine, which boosts alertness and energy. For most healthy adults, moderate coffee intake is generally safe and may have some health benefits, but too much can cause jitteriness, poor sleep, and a faster heartbeat. People who are pregnant, breastfeeding, or sensitive to caffeine should limit their intake and check with a doctor or pharmacist.
