Caffeine and Ephedrine, Hydroxyzine, Theophylline

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Caffeine interacts with whole categories of medication. Ephedrine, Hydroxyzine, Theophylline falls into 3 of those categories, so it can interact with Caffeine in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Major).

Major

EPHEDRINE

1 of 3
Theoretically, concomitant use might increase the risk for stimulant adverse effects.
Use of ephedrine with caffeine can increase the risk of stimulatory adverse effects. There is evidence that using ephedrine with caffeine might increase the risk of serious life-threatening or debilitating adverse effects such as hypertension, myocardial infarction, stroke, seizures, and death.
Severity Major Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Here's the thing to watch with this combo medication (which contains ephedrine and theophylline): both of those ingredients are stimulants in their own right. Caffeine is also a stimulant, so stacking it on top doesn't add a brand new interaction so much as it piles more fuel on the same fire. Theophylline in particular has a narrow safety window, and caffeine is chemically very similar to it, so they can push each other's effects higher.

In real life that can mean a pounding or racing heart, jitteriness, trouble sleeping, anxiety, and higher blood pressure. The serious warnings (things like dangerous blood pressure spikes) come mostly from anecdotal reports, so don't panic, but it's worth being cautious. Please check with your pharmacist or doctor about how much coffee, tea, or energy drinks is okay for you.

STIMULANT DRUGS

2 of 3

Why this applies to Ephedrine, Hydroxyzine, Theophylline

Ephedrine, Hydroxyzine, Theophylline falls under STIMULANT DRUGS — the group Caffeine interacts with.

Theoretically, concomitant use might increase stimulant adverse effects.
Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Severity Moderate Likelihood Probable Evidence C

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combo medicine packs two ingredients that already rev up your body: ephedrine (a stimulant similar to adrenaline) and theophylline (a bronchodilator that's chemically related to caffeine). Adding more caffeine on top, whether from coffee, energy drinks, or pills, can stack those stimulant effects. You may notice a racing or pounding heart, jitteriness, shakiness, trouble sleeping, headache, or feeling wired and anxious.

Theophylline matters most here. It has a narrow safety window, and too much can cause nausea, a fast heartbeat, or in serious cases tremors and seizures. This concern is based mainly on expert opinion, not strong studies, so don't panic. Still, it's smart to go easy on extra caffeine and check with your pharmacist or doctor about a safe amount for you.

THEOPHYLLINE

3 of 3

Ephedrine, Hydroxyzine, Theophylline also falls under THEOPHYLLINE.

Theoretically, caffeine might increase the levels and adverse effects of theophylline.
Large amounts of caffeine might inhibit theophylline metabolism.

This combination product contains theophylline, and that is the piece caffeine can tangle with. Caffeine and theophylline are chemical cousins, and they are both broken down by the same liver pathway. When you drink a lot of caffeine, it can slow down how fast your body clears theophylline, letting it build up a bit higher than expected.

Theophylline already has a narrow safe range, so even small bumps can matter. Higher levels can mean more jitteriness, a racing or pounding heart, nausea, trouble sleeping, and feeling wired or shaky, which is also worsened by the ephedrine in this product. This is based mostly on expert opinion, not strong studies, and an occasional cup of coffee is unlikely to cause trouble. Still, please check with your pharmacist or doctor before relying on lots of caffeine.

Caffeine is a natural stimulant found in coffee, tea, and many other plants and products.
