PHENYTOIN (Dilantin)
Animal research suggests that caffeine can decrease the anticonvulsant activity of phenytoin. The effect does not seem to be related to the seizure threshold-lowering effects of caffeine. However, the exact mechanism of this interaction is unclear.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
If you take phenytoin (Dilantin) to keep seizures under control, here's the gist on caffeine. There's a theory that caffeine could blunt how well phenytoin works, which in theory might make seizures more likely to break through. Phenytoin isn't a prodrug, so this isn't about caffeine speeding up or slowing down how you clear it. The exact reason isn't well understood.
I want to be honest about how shaky this is: the concern comes from animal studies only, not from people. So a normal cup or two of coffee likely isn't going to cause a problem. Still, phenytoin is a drug where steady levels really matter, so if you're loading up on energy drinks or high-dose caffeine pills, mention it to your pharmacist or doctor first.

What Is Phenytoin Used For?
Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection
Phenytoin treats seizures. Oral products are used for generalized tonic-clonic (grand mal) seizures and for psychomotor (temporal lobe) or complex partial seizures. Most oral products are also used to prevent and treat seizures during or after neurosurgery.
Phenytoin Sodium Injection is used for generalized tonic-clonic status epilepticus (a prolonged, dangerous seizure emergency). It is also used for seizures during neurosurgery. The injection should be used only when taking phenytoin by mouth is not possible. Dilantin-125 and Phenytoin oral suspension are listed for tonic-clonic and psychomotor seizures.
