Caffeine and Norgestrel

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Caffeine interacts with whole categories of medication. Norgestrel falls into 2 of those categories, so it can interact with Caffeine in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS

1 of 2

Why this applies to Norgestrel

Norgestrel falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Caffeine interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Caffeine is metabolized by CYP1A2. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind: in this pairing, it's the caffeine that gets affected, not your birth control. The progestin in Norgestrel can gently slow down a liver enzyme called CYP1A2, which is the same enzyme your body uses to clear caffeine. So your coffee or tea may stick around a bit longer than usual, and you might feel more wired, jittery, have a faster heartbeat, or trouble sleeping.

That said, this is rated minor and is based only on lab-type data, not real patient reports, so for most people it's not a big deal. If you notice caffeine hitting you harder, simply cut back a cup. Your Norgestrel will still work to prevent pregnancy. Any concerns, just check with your pharmacist.

CONTRACEPTIVE DRUGS

2 of 2

Norgestrel also falls under CONTRACEPTIVE DRUGS.

Theoretically, contraceptive drugs might increase the levels and adverse effects of caffeine.
Oral contraceptives decrease the rate of caffeine clearance by 40-65%.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the thing to know about this pairing: it's the caffeine that gets affected, not your birth control. Norgestrel (a progestin) can slow down how fast your body clears caffeine, by a fair amount in some studies. So that same cup of coffee may hang around longer and hit a little harder than you're used to.

In real life, that might mean more jitteriness, a racing or pounding heart, trouble sleeping, or feeling wired if you drink a lot of coffee, tea, or energy drinks. The good news is this is rated minor and is partly theoretical. Your norgestrel keeps working fine for preventing pregnancy. If caffeine starts bugging you, just ease back on it and chat with your pharmacist.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Norgestrel page

“Norgestrel (Ovrette) is a progestin-only birth control pill, so the biggest danger on this page is not a strange side effect but a pill that quietly stops working. That risk makes this a page for genuine vigilance.”

About Caffeine

What is Caffeine?

  • Mental alertness and reducing fatigue
  • Improving athletic performance
  • Headache and migraine relief
  • Boosting focus and concentration
  • Weight loss aid

Caffeine is a natural stimulant found in coffee, tea, and many other plants and products. In moderate amounts it can boost alertness and reduce tiredness for most healthy adults, but too much can cause jitteriness, a racing heart, and sleep problems. Pregnant women and people with heart conditions or anxiety should be especially careful and check with a doctor or pharmacist.

About Progestin-Only (norethindrone) Oral Contraceptives

What Is Progestin-Only (norethindrone) Oral Contraceptives Used For?

Brand names: Ovrette, Orval 21

Norethindrone progestin-only pills — sometimes called the "mini-pill" — are used solely to prevent pregnancy. They do not contain estrogen, which makes them an option for women who cannot or prefer not to use estrogen-containing contraceptives.

When taken perfectly (same time every day, no missed pills), fewer than 1 in 100 women will become pregnant in the first year. With typical use, that number rises to about 9 in 100 — mostly because of late or skipped pills. Consistency is everything with this method.
