Why this applies to Selegiline

Selegiline falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Caffeine interacts with.

Theoretically, concomitant use might increase the risk of a hypertensive crisis.
Caffeine has been shown to inhibit monoamine oxidase (MAO) A and B in laboratory studies. Concomitant intake of large amounts of caffeine with MAOIs might precipitate a hypertensive crisis. In a case report, a patient that consumed 10-12 cups of caffeinated coffee and took the MAOI tranylcypromine presented with severe hypertension. Hypertension was resolved after the patient switched to drinking decaffeinated coffee.

Here's the gist with selegiline: it's an MAO inhibitor, and at higher doses it loses some of its selectivity for the brain enzyme it targets. The concern is that piling on lots of caffeine could stack the stimulant effects and, in theory, push your blood pressure up sharply. That's what we call a hypertensive crisis, and it can show up as a pounding headache, a racing heart, sweating, or feeling very flushed.

I want to be honest about the strength of this one: it's mostly theoretical, based on lab work and a single case involving a different, stronger MAOI and someone drinking 10 to 12 cups of coffee. A normal cup or two probably isn't a problem for most folks. Still, since blood pressure spikes are serious, it's worth running your usual caffeine habit by your pharmacist or doctor.

What Is Selegiline Used For?

Brand names: Carbex, Eldepryl, Emsam, Zelapar

Selegiline is used for two different conditions, depending on the product.

  • Selegiline hydrochloride tablets and capsules, and Zelapar (orally disintegrating tablet), are add-on treatments for Parkinson’s disease in people taking levodopa/carbidopa whose response to it has gotten worse.
  • Emsam (skin patch) treats major depressive disorder in adults.

For Parkinson’s disease, there is no evidence from controlled studies that selegiline helps without levodopa.
