Ephedrine Sulfate falls under STIMULANT DRUGS — the group Caffeine interacts with.
Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: both caffeine and ephedrine are stimulants, and they push your body in the same direction. Caffeine wakes up your brain by blocking adenosine, while ephedrine revs up your heart, blood pressure, and breathing. Stack them together and those effects can add up.
So if you're getting ephedrine (sometimes given in a hospital setting to support blood pressure, or used in some breathing products) and you're also drinking a lot of coffee or energy drinks, you may notice a racing or pounding heart, higher blood pressure, jitteriness, headache, or trouble sleeping. This is mostly expert caution rather than hard proof, and a normal cup of coffee is usually less of a worry than big doses. Still, mention your caffeine habits to your pharmacist or doctor so they can keep an eye on things.

Here's the simple version: both caffeine and ephedrine are stimulants, so pairing them tends to push your heart and nervous system in the same direction. Ephedrine itself revs up the heart and can raise blood pressure, and caffeine can add to that. The concern on file is that large amounts of caffeine may strengthen ephedrine's effect on how hard your heart pumps, which could mean a faster or pounding heartbeat, jitteriness, a spike in blood pressure, trouble sleeping, or feeling wired and anxious.
Keep in mind this is a theoretical caution based on how these two work, not a documented disaster. A normal cup of coffee is very different from heavy energy-drink or pill-strength caffeine. Just let your pharmacist or doctor know what you're taking so they can keep an eye on things.
