Here's the tricky part with this one: your Marax-type combo already contains ephedrine, plus theophylline, which is itself a stimulant that revs up the heart and lungs. Layering an ephedra supplement on top means stacking stimulant on top of stimulant. That can push your heart rate and blood pressure higher than intended, and it may bring on a racing or pounding heartbeat, jitteriness, trouble sleeping, headache, tremor, or nausea. With theophylline especially, the margin for safety is narrow, so even modest extra stimulation can matter.

The evidence here is mostly anecdotal and theoretical, but the risk is taken seriously because the harms can be major. Please don't add any ephedra-containing product to this medication. Talk with your pharmacist or doctor first about safer options.

Here's something important about this combination product: it already contains ephedrine (a stimulant) and theophylline (a methylxanthine that also revs up the heart and nervous system). Adding an ephedra supplement on top stacks two more doses of stimulant on what you're already taking. That's a real problem, because you'd basically be doubling up on the same kind of effect.

When stimulants pile up like this, you can see a racing or pounding heart, jitteriness, trouble sleeping, tremor, and dangerously high blood pressure. In serious cases this stacking has been tied to heart attack, stroke, seizures, and even death. The evidence here is largely anecdotal, but the risk is taken seriously. Ephedra is also banned in U.S. supplements. Please talk with your pharmacist or doctor before mixing anything like this.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the tricky part with this combo product: it already contains ephedrine, the same stimulant found in ephedra. Layering ephedra on top would basically be doubling up on a stimulant, which can spike your blood pressure and heart rate, cause jitteriness, palpitations, and in serious cases dangerous heart rhythms. That stacking risk worries me far more than the enzyme effect.

On the enzyme side, this is only a theory from animal studies. Ephedra may speed up the CYP1A2 enzyme that clears theophylline, which could lower theophylline levels and make it work less well for breathing. Theophylline has a narrow safety window, so any change matters. Please don't pair these without checking with your pharmacist or doctor first.
