Coffee and Levonorgestrel

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CONTRACEPTIVE DRUGS

Why this applies to Levonorgestrel

Levonorgestrel falls under CONTRACEPTIVE DRUGS — the group Coffee interacts with.

Theoretically, concomitant use might increase the effects and adverse effects of caffeine found in coffee.
Coffee contains caffeine. Oral contraceptive drugs can decrease caffeine clearance by 40% to 65%.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to keep in mind: this interaction works in one direction, and it's about the coffee, not the levonorgestrel. Hormonal contraceptives like the daily progestin pills and IUDs can slow down how fast your body clears caffeine, sometimes by 40% to 65%. So a cup of coffee may hit harder and stick around longer than usual.

For a one-time emergency pill like Plan B, this really isn't worth worrying about, since you're only taking it once. With ongoing levonorgestrel (like Mirena or a daily pill), you might just notice more caffeine jitters, a racing heart, or trouble sleeping. The fix is simple: ease up on the coffee if you feel wired. It does not make your birth control work any less. As always, check with your pharmacist if you're unsure.

About Coffee

What is Coffee?

  • Alertness and reducing fatigue
  • Mental focus and concentration
  • Athletic performance
  • Headache relief
  • Weight management

Coffee is a widely consumed beverage made from roasted coffee beans, valued mainly for its caffeine, which boosts alertness and energy. For most healthy adults, moderate coffee intake is generally safe and may have some health benefits, but too much can cause jitteriness, poor sleep, and a faster heartbeat. People who are pregnant, breastfeeding, or sensitive to caffeine should limit their intake and check with a doctor or pharmacist.
