These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Coffee interacts with whole categories of medication. Ephedrine Sulfate falls into 2 of those categories, so it can interact with Coffee in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
STIMULANT DRUGS
1 of 2Why this applies to Ephedrine Sulfate
Ephedrine Sulfate falls under STIMULANT DRUGS — the group Coffee interacts with.
Coffee contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: both your coffee and ephedrine are stimulants, so drinking them together is a bit like stepping on the gas twice. Ephedrine itself revs up your nervous system to raise blood pressure and open the airways, and caffeine adds its own push on top of that. The concern is that the two effects stack up.
When that happens, you may notice a racing or pounding heartbeat, jitteriness, anxiety, shakiness, trouble sleeping, or a higher blood pressure. Keep in mind this is mostly based on expert opinion rather than hard studies, so it's a theoretical add-on effect, not a guaranteed problem. Still, if you're using ephedrine and feel wired or your heart is pounding, ease up on the coffee and check in with your pharmacist or doctor.

BETA-ADRENERGIC AGONISTS
2 of 2Ephedrine Sulfate also falls under BETA-ADRENERGIC AGONISTS.
Coffee contains caffeine. Caffeine can increase cardiac inotropic effects of beta-agonists.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: both coffee and ephedrine are stimulants, so pairing them tends to push your heart and nervous system in the same direction. Caffeine can add to ephedrine's effects on how forcefully your heart pumps, which is why you might notice a faster or harder heartbeat, palpitations, jitteriness, restlessness, a rise in blood pressure, or trouble sleeping. Ephedrine works on its own (it isn't a prodrug that needs activating), so this is really about two stimulants stacking up.
To be honest, this concern is theoretical, based on how these compounds behave rather than strong studies. A normal cup or two of coffee likely won't cause trouble, but loading up on caffeine while using ephedrine is where I'd be cautious. Please run your specific situation by your pharmacist or doctor.

About Coffee
What is Coffee?
- Alertness and reducing fatigue
- Mental focus and concentration
- Athletic performance
- Headache relief
- Weight management
Coffee is a widely consumed beverage made from roasted coffee beans, valued mainly for its caffeine, which boosts alertness and energy. For most healthy adults, moderate coffee intake is generally safe and may have some health benefits, but too much can cause jitteriness, poor sleep, and a faster heartbeat. People who are pregnant, breastfeeding, or sensitive to caffeine should limit their intake and check with a doctor or pharmacist.
