These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Coffee interacts with whole categories of medication. Aspirin, Caffeine, Codeine Phosphate falls into 2 of those categories, so it can interact with Coffee in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
ANTICOAGULANT/ANTIPLATELET DRUGS
1 of 2Why this applies to Aspirin, Caffeine, Codeine Phosphate
Aspirin, Caffeine, Codeine Phosphate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Coffee interacts with.
Coffee contains caffeine. Caffeine is reported to have antiplatelet activity. Theoretically, the caffeine in coffee might increase the risk of bleeding when used concomitantly with these agents. However, this interaction has not been reported in humans. There is some evidence that caffeinated coffee might increase the fibrinolytic activity in blood.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's an interesting wrinkle with this combination product: it already contains caffeine, the same active compound found in your coffee. So when you sip a cup alongside it, you're stacking caffeine on caffeine. The bigger picture our database flags is bleeding. Aspirin is a blood thinner that keeps platelets from clumping, and caffeine is reported in lab studies to have a mild antiplatelet effect of its own. In theory, the two together could slightly raise your chance of bruising, nosebleeds, or stomach bleeding.
That said, this is theoretical and based only on test-tube and animal data, not real people, so I wouldn't lose sleep over a normal cup or two. Just watch for unusual bruising or stomach upset, and check with your pharmacist if you take this often.

STIMULANT DRUGS
2 of 2Aspirin, Caffeine, Codeine Phosphate also falls under STIMULANT DRUGS.
Coffee contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing worth noticing: this medication already contains caffeine right in the formula, along with aspirin and codeine. So when you drink coffee on top of it, you're stacking caffeine on caffeine. That extra stimulant load is what this interaction is flagging.
In real life, that can mean feeling jittery, a racing or pounding heart, trouble sleeping, restlessness, or even an upset stomach (and aspirin can already be rough on the stomach). This is more of a theoretical, common-sense caution than a dangerous combo, and a single cup probably won't cause trouble for most people. But several cups could push you over the edge, especially if caffeine usually makes you wired. If you notice your heart racing or you can't settle down, that's worth a quick chat with your pharmacist or doctor.

“Anacin with Codeine combines aspirin, caffeine, and codeine, and each of the three picks up its own supplement problems, so this is a list worth real caution. Just over half of the 398 ingredients here, 202 of them, can add to aspirin's blood-thinning action and raise bleeding risk.”
