Here is the interaction we found between these two, with the evidence behind it.
ANTIHYPERTENSIVE DRUGS
Why this applies to Iloprost
Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group Stevia interacts with.
Stevia extract and stevioside might lower blood pressure in patients with hypertension. However, other clinical research suggests that stevioside does not significantly lower blood pressure in patients with hypertension.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with stevia and your inhaled iloprost (Ventavis). Iloprost already works by relaxing and widening your blood vessels to ease pressure in your lungs. The idea on file is that stevia might nudge blood pressure down a little too. Stack those two together and, in theory, you could feel more of iloprost's usual effects like flushing, dizziness, headache, or feeling lightheaded when you stand up.
I want to be honest though: this is a theoretical, minor concern based on how things work on paper, and the human studies on stevia and blood pressure don't even agree. Everyday food and drink amounts of stevia are very unlikely to cause trouble. If you're using concentrated stevia supplements, just mention it to your pharmacist or doctor first.

About Stevia
What is Stevia?
- Sugar substitute / sweetener
- Blood sugar support in diabetes
- Weight management
- Blood pressure support
- Reducing calorie intake
Stevia is a plant-based, calorie-free sweetener that is widely used as a sugar alternative and is considered safe in normal food amounts by major regulators. Purified stevia extracts have a good safety record, but evidence for medicinal benefits like lowering blood sugar or blood pressure is limited. Talk to your pharmacist or doctor before using stevia supplements for any health condition.

Iloprost Is Linked to Lower Levels of One Nutrient
Natural Medicines records Iloprost as a medicine that can lower these. This is not an interaction with Stevia — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
