Stevia and Enalapril Maleate

Here is the interaction we found between these two, with the evidence behind it.

Minor

ANTIHYPERTENSIVE DRUGS

Why this applies to Enalapril Maleate

Enalapril Maleate falls under ANTIHYPERTENSIVE DRUGS — the group Stevia interacts with.

Theoretically, combining stevia or stevia constituents with antihypertensive agents might increase the risk of hypotension.
Stevia extract and stevioside might lower blood pressure in patients with hypertension. However, other clinical research suggests that stevioside does not significantly lower blood pressure in patients with hypertension.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Good news first: if you enjoy stevia in your coffee or a drink, the everyday amount you'd use as a sweetener is very unlikely to affect your enalapril at all. This is a minor, mostly theoretical concern based on how the body might respond, not on strong studies.

The idea is that concentrated stevia supplements (not the sprinkle on your food) might nudge your blood pressure down a little on their own. Since enalapril already lowers blood pressure, stacking the two could in theory add up and leave you feeling dizzy, lightheaded, or tired, especially when standing up quickly. Honestly, the research here is mixed, and some studies show stevia doesn't move blood pressure much. If you're thinking about taking stevia in supplement form, just check with your pharmacist or doctor first.

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Enalapril Maleate page

“Potassium is the supplement I worry about first with enalapril maleate (Vasotec, Epaned). This ACE inhibitor, used for high blood pressure and heart failure, already makes the body hold onto potassium, so adding more can raise blood levels too far, a problem called hyperkalemia.”

About Stevia

What is Stevia?

  • Sugar substitute / sweetener
  • Blood sugar support in diabetes
  • Weight management
  • Blood pressure support
  • Reducing calorie intake

Stevia is a plant-based, calorie-free sweetener that is widely used as a sugar alternative and is considered safe in normal food amounts by major regulators. Purified stevia extracts have a good safety record, but evidence for medicinal benefits like lowering blood sugar or blood pressure is limited. Talk to your pharmacist or doctor before using stevia supplements for any health condition.

About Enalapril

What Is Enalapril Used For?

Brand names: Vasotec, Epaned

Enalapril is used to treat high blood pressure (hypertension) in adults and children older than one month, and to treat symptomatic heart failure. Lowering blood pressure reduces the risk of stroke and heart attack.

It is also used for asymptomatic left ventricular dysfunction — when the heart's pumping ability is reduced but the person doesn't yet have obvious heart failure symptoms — to slow the progression to overt heart failure and reduce hospitalizations. Enalapril maleate oral solution (including Epaned) and enalapril maleate tablets (including Vasotec) share these three approved uses.
