Here's the gist of this one. Perphenazine (Trilafon) belongs to a group of medicines that can slightly lower your seizure threshold, meaning they make seizures a bit more likely in people who are prone to them. The concern with melatonin is mostly theoretical, but some reports suggest melatonin may raise seizure frequency in certain people, especially children with neurological conditions.

For most adults using melatonin short-term for sleep, this combo is unlikely to cause a problem. The worry grows if you already have a seizure history or epilepsy. This isn't about melatonin changing how Perphenazine is broken down. It's about two things that can each nudge seizure risk being used together. Please check with your pharmacist or doctor before adding melatonin, especially if seizures are a concern for you.

Perphenazine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, melatonin might increase levels of drugs metabolized by CYP2D6.
Laboratory research suggests that certain lots of melatonin inhibit CYP2D6. Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2D6 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for these two. Your liver uses an enzyme called CYP2D6 to help clear perphenazine out of your system. Some lab studies hint that melatonin might slow that same enzyme down a bit. In theory, that could let perphenazine build up a little higher than usual, which might nudge up its side effects, things like daytime drowsiness, dizziness, dry mouth, or feeling unusually stiff or restless.

I want to be honest about how soft this is, though. This is based only on test-tube research, not real patients, and it's rated as a minor, theoretical concern. For most people it won't cause trouble. Since both can make you sleepy, just keep an eye on grogginess and check with your pharmacist or doctor before combining them.

The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.

What Is Perphenazine Used For?

Brand name: Trilafon

Perphenazine tablets are approved to treat schizophrenia in adults. They are also used to control severe nausea and vomiting in adults when other measures are not enough.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
