Here is the interaction we found between these two, with the evidence behind it.
IMMUNOSUPPRESSANTS
Why this applies to Rituximab-Abbs
Rituximab-Abbs falls under IMMUNOSUPPRESSANTS — the group Melatonin interacts with.
Melatonin can stimulate immune function. Theoretically, melatonin might interfere with immunosuppressive therapy.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this pairing. Truxima (rituximab-abbs) is a medicine designed to quiet down part of your immune system by targeting certain B-cells, and doctors use it for conditions like certain lymphomas, rheumatoid arthritis, and some autoimmune diseases. Melatonin, on the other hand, can gently nudge the immune system to be a little more active. So in theory, melatonin could push in the opposite direction of what your rituximab treatment is trying to do, possibly making it work less effectively.
I want to be honest with you: this is mostly a theoretical concern, and the evidence behind it is limited. It is not a guaranteed problem. Still, since rituximab is serious therapy, please check with your oncologist, rheumatologist, or pharmacist before adding melatonin so they can weigh in for your situation.

From our pharmacist’s overview on the Rituximab-Abbs page
“If a supplement promises to boost the immune system, chances are it appears somewhere on this page. Rituximab-Abbs (Truxima) works by quieting part of the immune response, clearing the B cells that drive certain cancers and autoimmune conditions, so every interaction here is the same basic conflict, an immune-stimulating ingredient tha…”

About Melatonin
What is Melatonin?
- Trouble falling asleep (insomnia)
- Jet lag
- Delayed sleep phase syndrome
- Non-24-hour sleep-wake disorder
- Shift-work sleep problems
- Sleep problems in some children with conditions like autism or ADHD
Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.
