Why this applies to Pirfenidone

Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP1A2. Also, other CYP1A2 substrates might decrease the metabolism of melatonin, increasing melatonin levels.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with drugs metabolized by the CYP1A2 enzyme might reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP1A2 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for your lung medication, Esbriet (pirfenidone). Your liver leans on an enzyme called CYP1A2 to clear pirfenidone out of your system. Melatonin is also handled by that same enzyme, so in theory the two could compete. If melatonin slows that pathway down even a little, pirfenidone levels might creep up, which could mean more of its usual side effects like nausea, stomach upset, tiredness, dizziness, or skin that's extra sensitive to sunlight.

Now, the honest part: this is a theoretical concern based on how the drugs are processed, not on real patient reports, so I don't want you to panic. Pirfenidone isn't a prodrug, so the worry is more drug buildup, not lost effect. Before adding melatonin, just run it past your pharmacist or doctor so they can keep an eye on how you're feeling.

Pirfenidone — available as capsules and tablets under the brand name Esbriet, as well as generic pirfenidone products — is indicated for the treatment of idiopathic pulmonary fibrosis (IPF), a chronic, progressive disease in which scar tissue builds up in the lungs and makes breathing increasingly difficult.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
