Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Resveratrol interacts with.

Theoretically, resveratrol might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that resveratrol can inhibit CYP1A2 enzymes. However, this interaction has not been reported in humans.

Here's the gist of it: your body relies on a liver enzyme called CYP1A2 to clear pirfenidone (Esbriet) out of your system. In lab studies, resveratrol appears to slow that same enzyme down. If that played out in real life, pirfenidone could build up a bit more than usual, which might mean stronger side effects like nausea, stomach upset, tiredness, dizziness, or more sun sensitivity.

I want to be honest about the evidence here: this comes only from test-tube research, and it has not been seen in actual patients. So it's a theoretical concern, not a proven one. Still, because pirfenidone can already be hard on the stomach, it's worth a quick chat with your pharmacist or doctor before adding a resveratrol supplement.

Pirfenidone — available as capsules and tablets under the brand name Esbriet, as well as generic pirfenidone products — is indicated for the treatment of idiopathic pulmonary fibrosis (IPF), a chronic, progressive disease in which scar tissue builds up in the lungs and makes breathing increasingly difficult.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
