Here's the piece that matters for this combo pill: the caffeine in your Fioricet with Codeine is cleared by a liver enzyme called CYP1A2. In lab and animal studies, lentinan seems to speed that enzyme up, which could make your body break caffeine down a little faster. The most likely real-world result, if anything happens at all, is that the caffeine wears off sooner, so it may feel slightly less effective at that mild pick-me-up and headache-helping role.
I want to be honest about how thin this evidence is. This has only been seen in test tubes and animals, never reported in actual people, so I wouldn't lose sleep over it. The codeine, butalbital, and acetaminophen parts aren't the concern here. Still, run it by your pharmacist or doctor before adding lentinan.

Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.

Here's the wrinkle with this combo product: the codeine part is a prodrug. Your liver uses an enzyme called CYP2D6 to convert codeine into morphine, which is what actually relieves pain. Lab research hints that lentinan might speed up that enzyme. If that happened in real life, codeine could be converted faster, which sounds like more pain relief but it could also mean more morphine-type side effects (drowsiness, constipation, nausea, slowed breathing) or, in some people, less steady pain control.
I want to be honest about the strength of this: it comes only from test-tube and animal studies and hasn't been seen in actual patients, so it's mostly theoretical. The acetaminophen, butalbital, and caffeine pieces aren't really the concern here. Still, before pairing lentinan with this medicine, run it by your pharmacist or doctor.

Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.

This combination pill is a mix of four ingredients, and the lentinan question really centers on how a liver enzyme called CYP3A4 handles them. Lab studies (not human studies) hint that lentinan might speed up CYP3A4. If that happened in real life, the butalbital portion and, in part, the codeine could be cleared a bit faster, which might mean weaker headache relief or less drowsiness than you'd expect.
Here's the catch: codeine is a prodrug that the body turns into morphine, mainly through a different enzyme (CYP2D6), so this CYP3A4 effect is a minor side path for it. Honestly, this is theoretical and based only on test-tube work, so I wouldn't lose sleep over it. Still, run it by your pharmacist or doctor before adding lentinan.
