Lentinan + Mirtazapine
Mirtazapine brand name: Remeron.
Mirtazapine falls into 3 drug categories that Lentinan can interact with; the most serious is rated moderate. Laboratory evidence suggests that lentinan might increase the activity of cytochrome P450 1A2 (CYP1A2).

These two interact in 3 distinct ways. Each one is explained separately below.
Why Are There 3 Interactions?
Lentinan interacts with whole categories of medication. Mirtazapine falls into 3 of those categories, so it can interact with Lentinan in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
1 of 3Why this applies to Mirtazapine
Mirtazapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Lentinan interacts with.
Some drugs metabolized by CYP1A2 include amitriptyline (Elavil), haloperidol (Haldol), ondansetron (Zofran), propranolol (Inderal), theophylline (Theo-Dur, others), verapamil (Calan, Isoptin, others), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist of it: your body uses a liver enzyme called CYP1A2 to help break down mirtazapine. Lentinan, in lab and animal tests, seems to speed up that enzyme. If that played out in real life, mirtazapine could be cleared a bit faster, which might leave you with lower levels and a weaker effect, so your depression symptoms could creep back, or you might notice less of the sleepiness mirtazapine usually brings.
That said, I want to be honest: this has only been seen in test-tube and animal studies, never reported in actual people, so it's more of a theoretical concern than something I'd lose sleep over. Mirtazapine isn't a prodrug, so this would weaken it rather than make it stronger. Before adding lentinan, just run it by your pharmacist or doctor.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
2 of 3Mirtazapine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
Some drugs metabolized by CYP2D6 include tricyclic antidepressants such as imipramine (Tofranil) and amitriptyline (Elavil); antipsychotics such as haloperidol (Haldol), risperidone (Risperdal), and chlorpromazine (Thorazine); beta-blockers such as propranolol (Inderal), metoprolol (Lopressor, Toprol XL), and carvedilol (Coreg); tamoxifen (Nolvadex); and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for mirtazapine (Remeron): your liver uses an enzyme called CYP2D6 as one of the ways it clears this drug. Lab and animal research hints that lentinan might speed that enzyme up a bit. If that held true in people, your body could break mirtazapine down a little faster, which in theory might lower its levels and make it work less well. You could notice your depression or sleep symptoms creeping back, or simply feel like the medicine isn't doing as much.
The reassuring part: this has only been seen in test-tube and animal studies, never confirmed in actual patients, so I wouldn't lose sleep over it. Mirtazapine also has backup pathways for being cleared. Still, before adding any lentinan supplement, run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
3 of 3Mirtazapine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Mirtazapine (Remeron): your liver uses an enzyme called CYP3A4 to help break this medicine down. Lab research hints that lentinan might speed up that enzyme. If that happened in real life, your body could clear mirtazapine a little faster, which in theory might leave less of it working and weaken its effect on your mood or sleep.
The big thing to know is that this is only seen in test-tube and animal studies, not in actual people, so I wouldn't lose sleep over it. Mirtazapine isn't a prodrug, so it doesn't flip the other way. If you take both and notice your depression slipping or your sleep getting worse, don't change anything on your own. Just check with your pharmacist or doctor first.

- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Mirtazapine page
“Depression and poor sleep tend to travel together, which is why people on mirtazapine (Remeron) so often eye herbal mood and sleep remedies, and that overlap is exactly where this list demands real care. Four of the 212 ingredients on file carry a major rating.”

About Lentinan
What is Lentinan?
- Immune support
- Used alongside cancer treatment
- Anti-inflammatory
- General wellness/tonic
- Antimicrobial support
Lentinan is a beta-glucan polysaccharide from shiitake mushrooms that is studied mainly as an immune-modulating add-on to cancer chemotherapy, where it is given by injection in a few countries. Evidence for oral supplements is limited, and quality varies widely between products. Talk with your pharmacist or doctor before using it, especially if you have a health condition or are pregnant or breastfeeding.
About Mirtazapine
What Is Mirtazapine Used For?
Brand name: Remeron
Mirtazapine is used to treat major depressive disorder (MDD), commonly called clinical depression. The mirtazapine tablet and orally disintegrating tablet labels, including Remeron and RemeronSolTab, state use in adults.
In studies, it worked in six-week trials, and one trial showed the benefit lasted for up to 40 weeks in people who had already responded. Doctors should periodically re-check whether longer-term use is still helping.

Mirtazapine Is Linked to Lower Levels of One Nutrient
Natural Medicines records Mirtazapine as a medicine that can lower these. This is not an interaction with Lentinan — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
